| Shrnutí: | Ošetřili jsme 20 pacientů s poraněním Achillovy šlachy 2 incizní operační technikou podle Wapnera - transferem šlachy flexor hallucis longus. Modifikovali jsme způsob ukotvení šlachy do patní kosti. Pacienty jsme sledovaly 1 rok od operace. AOFAS Hindfoot score vzrostlo z původní předoperační průměrné hodnoty 27 k výsledné průměrné hodnotě 93. S celkovým výsledkem operace bylo spokojeno 15 pacientů. Modifikovanou Wapnerovu techniku transferu šlachy flexor hallucis longus lze považovat za vhodnou alternativu pro řešení rozsáhlých ruptur Achilovy šlachy. V průběhu několika operací došlo opakovaně k diskrepanci mezi lokálním nálezem a běžně popisovanou anatomickou strukturou v oblasti křížení šlach flexor hallucis longus a flexor digitorum longus. Zkoumali jsme teda tuto oblast na kadaverech a hledali jsme orientační bod na noze, vzhledem k existujícím anatomickým strukturám, vhodný pro miniinvazivní preparaci Henryho uzlu. We changed the method of the fixation transferred tendon to the calcaneus. Follow up 6 months. The AOFAS Hindfoot score outcomes grown from average 27 to 93 in 1 year after the surgery. Subjectively, 15 patients were satisfied with final effect of the surgery. Modified Wapner technique of flexor hallucis longus tendon transfer is sufficient alternative for treatment of neglected Achilles tendon rupture. During the surgeries we noticed a discrepancy between local structure and the standardly described anatomical relationship of flexor hallucis longus and flexor digitorum longus tendons. We studied this structure on cadaver legs and tried to find a landmark on the foot due to the existing anatomical structures for minimally invasive preparation of the Henry`s knot.
